She has also made a handful of guest appearances on All My Children in 2004 playing the Kelly Cramer character. She is the third actress to portray Kelly Cramer Buchanan. Tom has also worked extensively in California theater.
She won two Daytime Emmy awards in 1993 and 1999 for "Outstanding Younger Lead Actress" along with nominations in 1994, 1995, 1996, 1997, 1998, 2000, 2004 and 2005. Heather Tom holds the record for the most Emmy Award nominations for actresses under the age of thirty.

Tom is a vocal supporter of reproductive rights for women, and helped found a group for daytime actors who are members of Planned Parenthood. In July 2005 Tom penned an op-ed in Soap Opera Digest criticizing Days of Our Lives for a storyline in which an abortion caused a young woman to develop a fertility-destroying infection.
